Terms and Conditions for FloLabs

FloLabs ("we," "us," "our") provides mobile laboratory collection services for physician-ordered laboratory requisition tests. By using our services, you agree to the following terms and conditions. These terms are subject to updates, and your continued use of our services constitutes your acceptance of any changes.
1. Service Terms
FloLabs provides mobile laboratory collection services to collect specimens at the client’s location and deliver them to the designated laboratory.
2. Charges and Fees
a. Service Fee
You may be charged a "Service Fee" for the services outlined at the time of booking or as displayed through our booking platform.
b. Late Cancellation/Reschedule Fee
Appointments may be canceled or rescheduled through our booking platform up to 24 hours before your scheduled appointment time.
Appointments canceled or rescheduled less than 24 hours from the scheduled appointment time will incur a $50 late fee.
c. No Show and Non-Compliance Fee
You may be charged the full Service Fee if:
You fail to show up on time for your appointment.
You fail to provide a valid requisition.
You are unprepared for the test (e.g., not fasting if required).
You are unavailable at the time of the appointment.
Unsafe or unfit conditions are present at the collection location.
d. GST/PST/HST
All amounts payable are subject to applicable goods and services taxes, provincial taxes, or harmonized taxes (GST/PST/HST).
e. Additional Surcharges
Additional charges may apply for travel, parking, or other unforeseen circumstances related to your appointment.
The services initial booked vary from the services required.
The amount of people requiring lab collections was not accurate at the time of booking.
3. Recollection Policy
If a recollection is required due to an error on FloLabs' part, we will provide the recollection at no additional charge.
FloLabs is not responsible for recollections required due to errors by the laboratory or third-party issues.
4. Appointment Requirements
A valid and legible paper requisition issued by a licensed physician must be provided at the time of service. Failure to provide this may result in cancellation of the appointment and full service charges.
Clients are expected to follow preparation instructions for their specific tests (e.g., fasting requirements).
5. Right of Refusal
FloLabs reserves the right to refuse service if the environment is deemed unsafe or unsuitable. Examples include:
Threatening or abusive behavior.
Unsafe physical conditions (e.g., poor maintenance, lack of proper lighting).
Presence of pests, hazardous materials, or unsecured pets.
Smoking or use of illegal substances during the appointment.
If service is refused for these reasons, the full service fee will still be charged.
6. Health and Safety
FloLabs is committed to ensuring the health and safety of our clients and employees. If any issues arise that compromise safety or hygiene, services may be delayed, rescheduled, or canceled.
7. Payment Processing
Payment information is required at the time of booking.
We accept credit card payments through secure third-party payment processors.
Payment will be processed once your appointment has been completed or if a late cancellation fee applies.
FloLabs does not accept cash or on-site payments unless confirmed and agreed to in writing by the FloLabs team prior to your scheduled appointment.
8. Laboratory Results
FloLabs does not process or deliver laboratory results. Test results will be provided directly by the laboratory or ordering physician.
9. Liability
FloLabs is not liable for any delays, cancellations, or errors caused by third parties, including laboratories and ordering physicians.
10. Amendments
FloLabs reserves the right to update or modify these Terms and Conditions at any time. Changes will be effective immediately upon posting.

By using our services, you acknowledge that you have read, understood, and agree to these Terms and Conditions.
